To join our Design Trade Program, please complete this application and email it to us with the following trade credentials:

• W9 Federal ID form or official document showing EIN
• US Tax Exemption or Resale Certificate
• Business card or business letterhead
• Any current business or state professional license in a residential or commercial design business or hospitality industry.

Once your application has been received with the valid documentation you will receive an email indicating your application is being reviewed.
